CE Required Documents – LTC Administrators

Important note: A downloadable version and online version are available for all documents EXCEPT the credit claim form) 

  1. LTC Sign in Sheet (downloadable)  or Online LTC Sign In Sheet (select LTC (long term care)
  2. Participant Profile – March 7 (downloadable)   or Participant Profile online version
  3. Evaluation – March 7 downloadable copy or Program Evaluation Online Version
  4. LTC Certificate – please print and keep.  NOTE: The AGEC does not submit your attendance to your applicable board. Official CE credit (certificate) must be processed according to your applicable board’s policy regarding CE credits.

    Note to facilitators:  All program forms MUST be emailed to agec@uams.edu, or faxed to 501-603-1966, within 5 business days of the event.

    Note to online participants: Whether you use the downloadable forms to email or fax OR enter all information using online links, the deadline for completion is 5 business days.
