Long Term Care (LTC) CE Nov.29, 2017 (GGR)

CE Required Documents – LTC Administrators

Important note: A downloadable version and online version are available for all documents EXCEPT the credit claim form) 

  1. LTC Sign in Sheet (downloadable)  or Online LTC Sign In Sheet (select LTC/long term care)
  2. Participant Profile – Nov.29 (downloadable)   or Participant Profile online version
  3. Evaluation – Nov. 29 downloadable copy or Program Evaluation Online Version
  4. LTC Administrator Certificate  – print and save -CE hours credit is valid only upon receipt of all documents listed above to AGEC. 

In order to receive CE credit, the documents listed above Must be filled out completely, and returned to the facility coordinator, upon check out, on the day of the event.

OR

Online or offsite participants MUST email this form to WThomasson@uams.edu, or fax this form to 501-603-1966, within 5 business days of the event.  All other required paperwork must also be completed online or emailed/faxed with this CE form to receive credit.  All paperwork must be submitted within 5 working days of the seminar to receive CE Credit.
