Geriatric Grand Rounds – December 3, 2019

  1. LTC Sign in Sheet (downloadable)  or Online Sign In Sheet (select LTC)
  2. Participant Profile (downloadable)   or Participant Profile online version
  3. Evaluation – 12-3-19 (downloadable)  
  4. Email agec@uams.edu the completed documents and/or indicate they were completed online. Include your name and program date/title in the body of the text. 

All documents must be submitted to AGEC within 5 business days of live event. Certificate will be emailed to participant within 30 days of receiving all required documents to receive CEs.

If you used the online version of all documents, please send agec@uams.edu an email notifying us of your entry or email all scanned CE documents to agec@uams.edu.
