CE Required Documents – Pharmacy 

  1. Pharmacy – Sign in Sheet – MUST put time in and time out  OR Online Sign In Sheet (select PharmD)
  2. Participant Profile – (downloadable) OR Participant Profile online version
  3. Program Evaluation – (downloadable) OR  Program Evaluation online version 
  4. Pharmacist Credit Claim Form – must submit for credit  (see details below) 

Reminder: All PharmD participants must indicate the “time in” and “time out” on their sign in sheet (both sign in versions give this option)

Note: Credit Claim Form MUST be printed, then emailed, faxed, or mailed to AGEC. Online participants may submit all other required documents directly to AGEC through SurveyMonkey links.

In order to receive CE credit, the documents listed above must be filled out completely, and returned to the facility coordinator, upon check out, on the day of the event.

Facilitators may provide and collect paperwork, and send as a group attendance by email to Certificates will be provided after the AGEC has verified all documents for CE credit have been submitted.

Online or offsite participants MUST email this form to, or fax this form to 501-603-1966, within 5 business days of the event.  All other required paperwork must also be completed online or emailed/faxed with this CE form to receive credit.  All paperwork must be submitted within 5 working days of the seminar to receive CE Credit.

Accreditation Council for Pharmacy Education (ACPE)

These knowledge based activities will provide pharmacists up to 2.0 contact hours or 0.2 CEU. CE credit information, based on verification of live attendance and completion of the program evaluation, will be provided to NABP within 60 days after the activity completion.
