Pharmacy CE Nov. 14-16

UAMS Accreditation Statement: In support of improving patient care, University of Arkansas for Medical Sciences is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC) to provide continuing education for the healthcare team.

These knowledge based activities will provide pharmacists up to 2.0 contact hours or .02 CEU.  CE credit information, based on verification of live attendance and completion of the program evaluation, will be provided to NABP within 60 days after the activity completion.  Link to handouts – click here.

CE Required Documents – Pharmacy 

Important note: A downloadable version and online version are available for all documents EXCEPT the Pharmacist Statement of Attendance 

  1. Pharmacy – Sign in Sheet Day 1, Day 2, Day 3  OR Online Sign In Sheet (select PharmD – complete once for any or all days)  Pharmacy must include Time In and Time Out
  2. Participant Profile – Nov.14-16 (downloadable) OR Participant Profile online version
  3. Program Evaluation – Nov. 14-16 (downloadable) OR  Program Evaluation online version  
  4. Test – There are three tests (one for each of the day’s courses). If you did not attend all courses within a day, each test question gives the option to select “Did not attend this segment” as your answer. We will adjust the scoring for test according to whether you attended the specific course (topic) and that will not affect your score.
  5. Download and complete Credit Claim form, sign and return as instructed in step 6. 
  6. Email that online documents have been completed and attached signed credit claim form

Reminder: All PharmD participants must indicate the “time in” and “time out” on their sign in sheet (both sign in versions give this option)

Facilitators may provide participant a certificate and collect the remainder of their paperwork to submit to AGEC.

Note: Statement of Attendance MUST be printed and emailed, faxed or mailed to AGEC. Online participants may submit all required documents directly to AGEC.
